This electronic Check-Up to Go (e-CHUG) is a brief self-assessment that provides you with accurate, detailed and personalized feedback on your use of alcohol, and specific information and helpful resources in the University of Memphis community.

Please answer all questions and answer them honestly. ALL answers are CONFIDENTIAL. Your name is not attached to this form and no personally identifiable information from this survey will be stored. Answering each question accurately will give you realistic feedback regarding your use of alcohol.

You will be given an ID number. Save that number to return to e-CHUG in the future. To use the program you must enable both JavaScript and cookies in your web browser for the proper operation of this web site.
