Full-Time On-Line Only Student Immunization Exemption Request

Full-Time On-Line only students can request an exemption of the immunization requirements.  They must complete the excemption form and fax it to the Registrar’s Office at (901) 678-1425.  Please click HERE for the exemption of the immunization requirements form.
